ECG OF THE MONTH

A 69 year-old white male presented to our clinic for establishment of care. Upon questioning, he stated that several years previously he experienced lightheadedness, palpitations, and one episode of syncope. Because of the latter symptoms, an implantable cardioverter-defibrillator (ICD) had been placed. An ECG was obtained. What is the diagnosis?
